Michael Scofield (Miller) is in a battle against time to save his brother, Lincoln Burrows (Dominic Purcell), on Death Row for an assassination he says he did not commit. With appeal options exhausted and time running out, Michael holds up a bank to get himself incarcerated with his brother in the maximum security Fox River State Penitentiary.

Once inside, he begins to execute an elaborate plan to break out Lincoln - unbeknown to the authorities Michael was one of the original structural engineers who built the jail. Michael uses his ingenuity and intelligence to align himself with a disparate group of prisoners, including a former mob boss and a man some believe to be the infamous skyjacker D.B. Cooper. As part of his escape plan, Michael is faking diabetes to gain access to the prison infirmary - and meeting with prison doctor Dr. Sara Tancredi (Sarah Wayne Callies) certainly adds to its appeal. Overseeing the prison is the powerful Warden Pope (Stacy Keach), who forges a close, almost father-son relationship with the new convict.
